Please fill out the form completely, be Accurate, Brief, and Clear.  
List the Misconduct and then describe the Incident, indicate the restart. In the case of language, include specific words used.
Report only the actions leading up to the Sendoff. (This is not a legal brief.)
Describe the event in terms of the action - "It was a kick to the groin" - Don't pass judgment.

The 2nd Yellow Card Send Off Revisited. (12-5-08)
Please read the following and start using the procedure immediately.

In the world we normally operate in, a 2nd yellow card is a send off, a reduction  of players on the team for that game, and a one game sit out.

Welcome to the NCS version of this event.

Under NFHS Rules, a 2nd yellow card does not result in a reduction of players on the team for that game, the player is sent off, and,
it is highly recommended that the player sit out the next game.

The nature of the 1st yellow card is not a problem, it is what it is.
No change for the 1st yellow card, the card is displayed, player leaves the field until the next substitution opportunity, a sub is allowed.

BUT, under NCS interpretations, there are two categories of yellow cards for the 2nd yellow card violation.

There are 2nd yellow card violations which are defined by their nature to violate the NCS Ejection Policy, if they are the 2nd card for that player.
There are other 2nd yellow card violations which do not violate the NCS Ejection Policy.
(No 1st yellow cards violate the NCS Ejection Policy, no matter their nature.)

In both cases, the player leaves the field, the team does not play down a player.

If the 2nd yellow card is issued for a violation which is considered to violate the NCS Ejection Policy, the player has to sit out the next game.
If other 2nd yellow card violations are the 2nd yellow card issued, the player is allowed to play in the next match.

How do you know which is which?  You don't?  The AD and NCS will decide, you just need to cite the specific reason for the 2nd yellow card.

I have set up the Send Off Report to make it easier for you to give them the information.
I will not try to explain the difference between the violations.
Also, you will see a category just for Coaches and Staff.
In the meantime, you need to be specific about the reason for the 2nd yellow card.  Again, I am not concerned about the 1st yellow card.

If it is not on the list, select USB Other and give the reason in the Details.
